Sa nearest DOST office po magbabayad. :) and you will pay for 3 years nalang, kasi natapos mo na yung 2 years na pag hihirap :)

cmm na required ay:

1. Transcript of Records
2. Diploma
3. Yellow Form
4. Certificate of Employment

This post can help you po:

I'm new here and sorry I didn't browse through the earlier posts. May question lang ako. I used to be a DOST scholar but I only enjoyed it for three years. And as expected, nagkaproblema ako sa NBI nung nag apply ako. Now I am working for a Japanese comopany here in Manila. My problem is, the nature of our job requires us to secure a passport kasi minsan we may be deployed in Japan for projects. Since I am still serving the country kasi employed naman ako dito, do I need to go through the usual process na magserve dito sa Philippines or magbayad. E job ko nga service na rin yun e... What do I need to do? I am running out of time. I only have a month to process my papers. People I need your help thanks.

You only need a written consent from your company. :)

P.S.: may gtalk ka ba? hehehe.. mas magaan kasi Gtalk at mas marami ata kami Gtalk users here ^__^ you can also use your @dostscholars.org email on Gtalk ^__^

thank you jemme for the very prompt reply. Ah so I just need to secure that certification. The project duration kasi is around 4 months. Wala ba silang limit na number of days for the business trip? If I could secure one, how many days bago ko makuha yung approval nila?

Im on gtalk now, bytemasterxxx@gmail.com is my ID.

Blue_PDT_01_06

About how many days ang approval ng DOST, it's just less than a week pag naka kuha ka na ng certificate from your company. :)

Photocopy of your TOR
Photocopy of your Diploma
2pcs 2x2 ID
A completely filled up Blue Form
A completely filled up Yellow Form

I hope this helps..
